Lewis County Middle School is partnering with 4-H to provide Health Rocks! As part of this program, students are invited to complete a brief survey.

Purpose of the Survey:
The survey helps evaluate the effectiveness of the Health Rocks! curriculum and gather general information related to student knowledge and experiences.

Important Information:

• Participation is voluntary.
• The survey is anonymous. No names will be collected.
• Questions include general knowledge items, limited demographic information, and 4-H involvement.
